Your rights

Under HIPAA, the federal health privacy law, you can:

• See or get a copy of your health records
• Ask us to correct your health records
• Ask us to contact you in a particular way or at a particular address
• Ask us to limit what we use or share
• Get a list of those we have shared your information with
• Get a paper copy of the notice
• Choose someone to act for you
• File a complaint if you believe your privacy rights have been violated
